At Home Living

Adult Foster Care

At Home Living provides foster care to adults living with a range of physical and mental disabilities. Many of our clients are still able to live successfully in our residential care homes if they receive an appropriate level of assistance.

Our foster care services offer a home-based environment that includes meals and a private room. But we aren’t simply a place to live—we’re a place to call home. And at home, our residents find they get the right level of support and care they require, including:

  • Up to 24-hour supervision and meal preparation
  • Socialization and travel arrangements
  • Individualized home management tasks
  • Assistance setting up meetings, appointments and medical or social services
  • Assistance managing personal funds

BRIDGES to Success

Community Services

BRIDGES to Success is a community-based program that provides an individualized level of care. Through BRIDGES—which stands for Building Relationships and Independence through Determination, Guidance and Educated Supports—we offer a variety of supports, including:

  • Accompanying clients to appointments and social functions
  • Assisting with medication management
  • Building skills to live more successfully, such as cooking, meal planning, cleaning and time management
  • Delivering social skills, anger management, relationship building, assertiveness and symptoms management training
  • Providing meaningful day programming

Wesley Residence & Garden Terrace

Assisted Living

It is our mission to offer a warm assisted living setting with caring and educated staff where we provide personalized services and an exceptional standard of living. We currently offer assisted living communities in Duluth and Wrenshall, Minnesota. Each location offers customized living tailored to each individual, as well as a variety of unique amenities.

  • Wesley Residence in West Duluth has 43 private rooms.
  • Garden Terrace is a 10 private-room residence in Wrenshall.

We accept a variety of payment options, including elderly waiver funding and private pay.
